The Interstate Medical Licensure Compact (IMLC) is an agreement among several U.S. states designed to simplify the process of obtaining medical licenses across state lines. Established in 2017, this innovative licensing category provides a streamlined method for physicians to secure medical licenses through a single application process in any participating IMLC state. The Interstate Medical Licensure Compact Commission (IMLCC) also oversees the renewal of these multi-state licenses, enabling physicians to manage all renewals through one central entity.

The Compact’s mission is to enhance healthcare access, particularly for patients in underserved or rural regions. By enabling physicians to broaden their practice, the IMLC improves access to medical specialists and encourages the adoption of new medical technologies, such as telemedicine.

Qualifications for the Interstate Medical Licensure Compact (IMLC)

To take advantage of the ILMC, ensure you meet the eligibility criteria. There are two types of eligibility requirements: State of Principal Licensure (SPL) and general eligibility.

SPL Eligibility

To receive licensure through the Compact, you must:

  • Hold a full, unrestricted medical license in a state that is a member of the Compact; this is called your State of Principal License (SPL). 

In addition, at least one of the requirements below must apply:

  • Your primary residence is in the SPL (State of Principal License).
  • At least 25% of your practice of medicine occurs in the SPL.
  • Your employer is located in the SPL.
  • You use the SPL as your state of residence for U.S. federal income tax purposes.

To remain active within the Compact, you must always maintain your SPL status. You may re-designate your SPL to another state after you have received your initial qualification, as long as the state is a participant in the Compact.

General Eligibility

In addition to having basic SPL status, there are several general eligibility requirements. You must:

  • Have graduated from an accredited medical school or a school listed in the International Medical Education Directory
  • Have completed ACGME- or AOA-accredited graduate medical education (GME)
  • Have passed each component of the USMLE, COMLEX-USA, or equivalent in no more than three attempts for each component. Please note that Passing the Canadian Licentiate of the Medical Council of Canada, or LMCC, does not meet this requirement.
  • Hold a current specialty certification or time-unlimited certification by an ABMS or AOABOS board.

In addition, you must:

  • Not have any history of disciplinary actions toward your medical license
  • Not have any criminal history
  • Not have any history of controlled substance actions toward your medical license
  • Not currently be under investigation

You then apply through the IMLCC website, using your state of principal licensure (where you are primarily licensed) to verify your credentials. Once your eligibility is confirmed, your letter of qualification (LOQ) is sent to other participating states where you wish to practice, and they issue you licenses without requiring you to go through their full, traditional application processes.

The cost to participate in the compact is $700.00 plus the cost of a license(s) in any compact state where you want to practice. All fees are non-refundable. Upon obtaining licensure through the IMLC, you will be subject to the jurisdiction of the medical board in the state where the patient is located.

States currently accepting applications for expedited licensure

*Note: Alabama, Connecticut, and Vermont are not currently designated as “States of Principal Licensure” (SPL). While you can obtain a license for these states through the compact if you qualify via another SPL, none of these states serve as an SPL for licensing in other states. Please refer to the ILMCC website for more details.
